The Function Score Analysis is a clinical review tool that helps ensure a resident’s ADL baselines accurately reflect their current level of function.
Jana analyzes resident documentation and caregiver charting to identify differences between a resident’s documented care plan and actual ADL performance observed by caregivers.
This analysis reviews historical documentation to determine the most clinically appropriate baseline for the seven ADLs that make up the resident’s function score.
How to access and run a Function Score Analysis
To run a Function Score Analysis for a resident:
Open the resident profile.
Click the Function Score link.
Select Run Analysis.
The system will then review the resident’s documentation and charting history and display findings and recommendations.
How the Function Score Analysis works
The analysis:
• Reviews caregiver charting and resident documentation history
• Compares observed ADL performance to the documented care plan
• Identifies inconsistencies that may impact clinical accuracy
• Provides a recommendation to update the ADL baseline when appropriate
Supporting documentation from the care plan or other sources is shown to assist with clinical decision-making.
Clinical outcomes
In many cases, the analysis results in one of the following:
• The care plan is correct, and the ADL baseline should be updated
• The ADL baseline is correct, and the care plan should be updated in the EHR
Final actions require clinical judgment from nursing staff.
