Alerts indicate changes in a resident’s condition that are observed by caregivers while they are charting care.

These changes can include, but are not limited to:

  • Changes from a resident’s ADL baseline

  • Incontinence or changes in toileting patterns

  • Changes in weight or nutrition

  • Other notable changes in condition

Alerts are generated automatically based on caregiver documentation and are intended to bring important changes to the attention of clinical staff.

All alerts must be reviewed and resolved by a nurse to ensure residents continue to receive the highest level of care and appropriate follow-up.
